Das klinische Bild der CAP reicht von sehr milden Symptomen mit Fieber, Husten und Dyspnoe bis zu schweren Symptomen mit septischer Verlaufsform und Lungenversagen. Die ambulant erworbene Pneumonie zählt zu dem häufigsten Fokus für Sepsis und septischen Schock. Dabei hängt der Schweregrad auch von der lokalen und systemischen Immunantwort des Patienten ab.

* Tachypnoe, vermehrte Atemarbeit

* Desorientiertheit (insbesondere bei älteren Patienten)

Viele der Symptome sind unspezifisch [1] und haben einen niedrigen positiven prädiktiven Wert. Besonders bei älteren und immunsupprimierten Patienten können typische Symptome fehlen, die auf eine pulmonale Infektion hinweisen fehlen und verlangen besondere Aufmerksamkeit, um frühzeitig die Diagnose zu stellen.

Bei klinischem Verdacht auf eine CAP ist die Röntgen-Thoraxaufnahme für die Diagnosestellung sehr bedeutend.

Für die initiale Risikostratifizierung zur Bewertung des Schweregrades und des Letalitätsrisikos hat sich die Kombination aus CRB-65 Score [2], Evaluation des funktionellen Status und relevanter Komorbiditäten sowie Messung der Oxygenierung etabiliert [3].
