Die angefügte Tabelle gilt als Anhalt, entscheidend für die empirischen Therapieempfehlungen sind die regionale bzw. hausinterne Resistenzstatistik und Patientenfaktoren, wie Schweregrad der Erkrankung, Vorerkrankungen und das Risikoprofil für resistente Erreger

Im septischen Schock sollte eine Kombinationstherapie (z.B. mit Gentamycin, Chinolonen) erfolgen, um das Erregerspektrum bestmöglich zu erfassen. Diese sollte spätestens nach 48-72 h auf eine effektive Monotherapie deeskaliert werden.

Tal-Spiegelbestimmung Zielwert : 15-20mg/L

Bei Patienten in besonderen klinischen Situationen, adipösen Patienten oder resistenten Erregern können höhere Dosierungen, als in der Tabelle angegeben, erforderlich sein. In diesen Fällen ist es sinnvoll, infektiologischen Rat einzuholen.

Die Dosierung und Applikation der Antiinfektiva sollte anhand pharmakokinetischen/ pharmakodynamischen Prinzipien und spezifischen Medikamenteneigenschaften bei Patienten mit Sepsis oder septischem Schock optimiert werden. So können auf Intensivstationen nach Applikation einer Aufsättigungsdosis prolongierte Infusionsschemata für einige Betalaktame angewandt werden, bzw Dauerinfusionen, wenn eine Spiegelmessung möglich ist.

Bei Z.n. antiinfektiver Vortherapie sollte ggf ein Substanzklassenwechsel erfolgen.

Schnellstmöglich sollten Infektionsfoci chirurgisch saniert werden. Fremdkörper sollten, soweit möglich, entfernt werden.

Die kalkulierte antiinfektive Initialtherapie sollte nach plausiblem Erregernachweis angepasst werden, sobald die Antiinfektivaempfindlichkeit bekannt ist. Bei klinischer Verbesserung des Patienten innerhalb der ersten 72 h sollte auch ohne Erregernachweis eine initiale Kombinationstherapie auf eine Monotherapie deeskaliert werden.
In der Regel beträgt die Therapiedauer 7-10 Tage. Ausnahmen sind die S aureus Bakteriämie sowie die Candidämie (siehe dort), persistierende Infektionsfoci oder schlecht zugängliche Kompartimente. Bei zügiger Fokussanierung (z.B abdominelle, urogene Foci) kann die Therapie verkürzt werden (3-5 Tage nach Fokussanierung)
