Therapie

Merke: Das Gelenkempyem stellt einen klinischen Notfall dar.

Empirische antiinfektive Therapie

A. Ohne Gram-Präparat

• Empirische Therapie, ambulant erworben, ohne Primärfokus, keine Op
  • Ampicillin/Sulbactam 3x 2g/1g i.v. (Alternative bei schwerer ß-Laktam-Allergie: Levofloxacin 1 x 500mg)
• Primärfokus urogenital/gastrointestinal, ambulant erworben:
  • Piperacillin/Tazobactam 4 x 4/0.5g i.v. (Alternative Meropenem 3 x 1g i.v.)

B. Mit initialem Grampräparat:

• Gram-positive Kokken (ambulant, keine vor OP): Cefazolin 3 x 2g i.v.
• Gram-positive Kokken (nosokomial/health-care associated, post OP): Vancomycin (nach Dosierungsschema hier ein Verweis auf das Schema)
• Gram neg. Kokken: Ceftriaxon 1 x 2g i.v.
• Gram neg. Stäbchen: Piperacillin /Tazobactam 4 x 4/0.5g i.v.

Nach Tierbiss:

• Ampicillin/Sulbactam 3 x 2/1g i.v., gefolgt von Amoxicillin/Clavulansäure 3 x 875/125g p.o.
