Standardprophylaxe[11][12]

Tenofovirdisoproxil/Emtricitabin (Truvada® oder diverse Generika) 200/245mg 1x1

plus INI RAL (Isentress®) 400mg 2x1

oder RAL (Isentress®) 600mg 1x2

oder DTG (Tivicay®)* 50mg 1x1

*cave: Dolutegravir wegen V.a. Teratogenität kontraindiziert für Frauen im gebärfähigen Alter, bei denen eine Schwangerschaft nicht ausgeschlossen ist

Dauer der PEP: 4 Wochen

Basis- und Kontrolluntersuchungen

Beginn der Prophylaxe[13][14]

Die PEP sollte so früh wie möglich nach einer Exposition eingeleitet werden (nach Möglichkeit sogar innerhalb von 24 Stunden und besser noch innerhalb von 2 Stunden). Liegt die mögliche Exposition vor mehr als 72 Stunden, kann nach derzeitigem Kenntnisstand eine Prophylaxe nicht mehr empfohlen werden.

Abklärung akuter HIV-Infektion[15][16]

Eine akute HIV-Infektion soll bei fieberhaftem Krankheitsbild innerhalb von 2 Monaten nach Exposition bzw. nach Ende der Prophylaxe diagnostisch abgeklärt werden. Bei negativem HIV-Antikörper Nachweis könnte eine HIV-PCR notwendig sein.

Besonders verdächtig ist ein akutes Krankheitsbild innerhalb der ersten vier Wochen nach Exposition bzw. Ende der PEP.

Nach Ende der PEP sollte evtl. der Bedarf einer HIV-PrEP (HIV-Präexpositionsprophylaxe) evaluiert werden.
