Klinisches Bild

Das klinische Bild von gynäkologischen Infektionen wird geprägt durch Veränderungen des Scheidenausflusses, dem Fluor, durch knötchenförmige und flächige Rötungen sowie brennenden Schmerzen und Juckreiz.

Je nach Infektion benötigt der Erreger zudem eine Eintrittspforte und verursacht oftmals ein typisches klinisches Bild.

Leitsymptome

Grundlagen:

Gynäkologische Infektionen führen häufig zu einer Veränderung des physiologischen Fluors. Wichtige Unterscheidungskriterien sind:

  • Konsistenz (wässrig, klebrig, dünn, fest)
  • Geruch (fischartig, säuerlich, stinkend)
  • Ursache der Infektion (Bakterien, Pilze, Viren)
  • deutlich erhöhte Leukozytenzahl im Fluor

Der Verlauf einer Infektion der weiblichen Geschlechtsorgane kann asymptomatisch oder nur von geringer Ausprägung sein. Typische Beschwerden einer Infektion können sein:

  • Pruritus
  • Brennen beim Wasserlassen oder Geschlechtsverkehr
  • Schmerzen
  • Rubor
  • Schwellung

Das klinische Bild einer Infektion lässt sich oft durch folgende Erscheinungen einordnen:

  • veränderter Fluor
  • Ulkus
  • Pusteln/Bläschen

-       plötzlich eintretende Symptomatik

Vulvitis pustulosa -       Hautläsionen

-       Läsionen an Hautanhangsgebilden

-       Schlechte und übertriebene Hygiene

-       Adipositas

-       Pusteln, Knötchen

-       Juckreiz

-       Rötung, Schwellung

-       Dysurie

Vulvitis

-       Furunkel/Karbunkel

-       eingewachsene Haare

-       verstopfte Talgdrüsen

-       Immunsuppression

-       mangelnde Hygiene

Vulvitis

-       ß-hämolysierende Streptokokken der Gruppe A

è         Streptococcus pyogenes

-       meist digitale Übertragung vom Nasen-Rachen-Raum

-       mangelnde Hygiene

-       „häufigste“ bakterielle Vulvitis bei Kindern vor der Pubertät

-       leichte bis starke immer flächige Rötung

-       erinnert an Pilzinfektion

-       Schmerzen

Erythrasma -       Infektion des Stratum corneum

-       eher ältere Frauen befallen

-       begünstigend ist feucht-warmes Milieu (Hautfalten, Labia majora)

-       meist keine Beschwerden

-       durch Porphyrinbildung der Corynebakterien rotbraune, makulopapulöse Hautveränderungen

Kolpitis

-       Trichomoniasis

-       begünstigend ist feucht-warmes Milieu der Scheide -       fleckige Rötung, kann mal an Herpes erinnern

-       Brennen stärker als Juckreiz

-       evtl. Dysurie

Fluor:

-       grün-gelb, bis schaumig, dünnflüssig, evtl. klebrig

-       pH > 5

Kolpitis

-       A-Streptokokken

-       Schmierinfektion von oral

-       Sexualverkehr

-       nicht von anderen Kolpitiden unterscheidbar
Kolpitis

-       Staph. aureus

-       begünstigend ist feucht-warmes Milieu der Scheide -       äußerst selten

-       typische Symptome

Kolpitis

-       ohne Erregernachweis

-       begünstigend ist feucht-warmes Milieu der Scheide -       meist chronisch

-       gelblicher Fluor über langen Zeitraum

-       viele Behandlungsversuche

-       pH 4,5 – 6

Klinische Situationen

Infektionen können in jedem Alter auftreten und hängen maßgeblich von dem Grad der sexuellen Aktivität, Hygiene, Hormonstatus, Antibiotikatherapien, Stress, Schlafmangel und chronischen Krankheiten/Immunsupression ab.

Frauen mit einer Frühgeburt in der Anamnese zählen bei einer bakteriellen Vaginose zu den Hochrisikopatientinnen. Ebenfalls stellen rezidivierende Harnwegsinfektionen und Blutungsanomalien einen Hinweis auf eine BV dar und sollten großzügig untersucht werden.

Epidemiologie

Bartholinitis Am häufigsten sind Frauen im Alter von 19 bis 35 Jahren betroffen[1]
Bakterielle Vaginose Ca. 5% der Frauen die zu einer Vorsorgenuntersuchung kommen haben eine BV[2]

Prognose

Akute Infektionen lassen sich meist effektiv therapieren, wobei chronische Verläufe oftmals nur symptomatisch behandelt werden können, oder bei der Bartholinitis eine chirurgische Intervention mit Marsupialisation erforderlich machen können.
