DGI:Opportunistische Infektionen/Kryptokokkose/Prophylaxe und Prävention

Primärprophylaxe: Eine medikamentöse Primärprophylaxe wird nicht generell empfohlen[1][2]. In Gebieten mit hoher Zahl HIV-Infizierter, begrenzter Verfügbarkeit einer antiretroviralen Therapie und/oder Resistenzen gegen antiretrovirale Medikamente kann eine regelmäßige Serum CrAg Testung erfolgen

Sekundärprophylaxe: Die Sekundärprophylaxe(Erhaltungstherapie) bei Z.n. schwer verlaufender Kryptokokkose (z.B. disseminierte Kryptokkokose, ZNS-Kryptokokkose) kann nach einer Therapiedauerdauer von mindestens 12 Monaten unter folgenden Voraussetzungen beendet werden:

  • Bei HIV-Infektion: CD4-Zellzahl > 100/µl und HI-VL unter der Nachweisgrenze > 3 Monate[3][3].
  • Z.n. adäquater Induktions- und Konsolidierungstherapie.
  • Serum CrAg ≤1:512 oder nicht zunehmend[2].
  • Reduktion der medikamentösen Immunsuppression bei transplantierten Patienten, falls vertretbar.

HIV: Bei erneut sinkender CD4-Zellzahl < 100/µl sollte wieder mit einer Erhaltungstherapie (Sekundärprophylaxe) begonnen werden[3][3].
