Primärprophylaxe: Eine routinemäßige Primärprophylaxe wird nicht empfohlen[1][2].Ausnahme: Bei HIV-Patienten in hochendemischen Regionen mit CD4 < 200/µl, wenn kein CrAg-Screening verfügbar ist, kann eine universelle Primärprophylaxe mit Fluconazol 100 mg/Tag erfolgen. [3]

Sekundärprophylaxe: Die Sekundärprophylaxe (Erhaltungstherapie) bei Z.n. schwer verlaufender Kryptokokkose (z.B. disseminierte Kryptokkokose, ZNS-Kryptokokkose) kann nach einer Therapiedauer von mindestens 12 Monaten unter folgenden Voraussetzungen beendet werden:

  • Bei HIV-Infektion: CD4-Zellzahl > 100/µl und HI-VL unter der Nachweisgrenze [4].
  • Z.n. adäquater Induktions- und Konsolidierungstherapie.
  • Reduktion der medikamentösen Immunsuppression bei transplantierten Patient:innen, falls vertretbar.

HIV: Bei erneut sinkender CD4-Zellzahl < 100/µl sollte wieder mit einer Erhaltungstherapie (Sekundärprophylaxe) begonnen werden[5][6].
