Klinisches Bild

Leitsymptome

Die initialen Symptome sind unspezifisch und bestehen typischerweise aus Kopfschmerzen, Übelkeit, Lethargie bzw. Vigilanzminderung, Fieber, Meningismus und Photo- und Phonophobie. Je nach Katheterlage kommen neben einem möglichen Erythem und Verhärtung über dem Kathetersystem weitere Symptome hinzu:

ventrikuloperitonealer (VP) Shunt

  • Peritonitis
  • abdominalen Druckempfindlichkeit

ventrikulopleuraler Shunt

  • Pleuritis

ventrikuloatrialer (VA) Shunt

  • Bakteriämie
  • Glomerulonephritis

Epidemiologie

Die Inzidenz von Shuntinfekten wird zwischen 5 und 41% angegeben, wobei die intraoperative Infektionsrate unter 4% beträgt.

Die Infektinzidenz von externen Liquorableitungen wird mit ca. 11 / 1000 Kathetertage angegeben, dabei liegt die Inzidenz bei Kathetern, die bis zu 7 Tagen liegen, bei ca. 20 / 1000d, bei einer Liegedauer zwischen 7 und 10 Tagen bei ca. 13 / 1000d und ab 10 Tagen Liegedauer bei 8 / 1000d [1].
