DGI:Opportunistische Infektionen/Invasive Aspergillose/Klinisches Bild
Invasive Aspergillosen, Aspergillome und Otomykosen werden durch Pilze der Gattung Aspergillus ausgelöst. Häufigste Lokalität sind die Atemwege der Patient:innen. Klinisch zeigen sich pulmonale Aspergillosen mit einer Infektion des Lungengewebes oder lokalen Entzündungen im Bereich der oberen Atemwege, wie zum Beispiel die Tracheobronchitis und der Rhinosinusitis.[1][2][3][4]Bei schwerer, langanhaltender Einschränkung des Immunsystems kann es zu invasivem Wachstum mit Invasion in das Gefäßsystem kommen, was zu einer disseminierten Infektion mit Beteiligung aller Organe führen kann.
Klassische Symptome sind Fieber, trockener Husten, Hämoptysen und pleuritische Schmerzen.
Klassische Risikofaktoren sind eine hämato-onkologische Grunderkrankung, wie zum Beispiel eine akute Leukämie, das myelodysplastische Syndrom, eine aplastische Anämie, oder Knochenmarkversagen. Ebenso sind Patient:innen mit schwerer und langandauernder Neutropenie nach Chemotherapie oder nach hochdosierten Kortikosteroiden und die Einnahme von Immunsuppressiva (z.B. bei Autoimmunerkrankungen, Transplantationspatient:innen) sowie AIDS-Patient:innen, einem erhöhtem Risiko ausgesetzt.[5] Des Weiteren wurden genetische Prädispositionen, wie zum Beispiel die septische Granulomatose (CGD), oder das Fehlen von Mannose-bindendem Lektin, Pentraxin 3 (PTX3), Toll-like Rezeptor 4 oder Dectin-1 Polymorphismen identifiziert.[6][7][8][9] Weitere Risikofaktoren bei Patient:innen nach allogener Stammzelltransplantation sind Graft-versus-Host Disease (GvHD) und CMV-Reaktivierung/Erkrankung.[10] Bei Intensivpatient:innen wurden vermehrt pulmonale Aspergillosen bei Influenza und COVID-19 nachgewiesen.[11][12]
Bei Patient:innen mit akuter myeloischer Leukämie (AML) beträgt die Inzidenz 6.4% und bei Patient:innen mit akuter lymphatischer Leukämie (ALL) 3.8%.[13] Eine eindeutige Inzidenz bei Intensivpatient:innen mit Influenza oder Covid-19 assoziierter pulmonaler Aspergillose konnte bislang noch nicht ermittelt werden. [14][15]Inzidenzraten schwanken bei unterschiedlichen Zentren unterschiedlicher Länder zwischen 5-25%.
Die Prognose der invasiven Aspergillose war zur Zeit des Jahrtausendwechsel äußerst schlecht mit teilweise hohen Letalitätsraten (50-80%). [16] Mit der Neuentwicklung von Antimykotika und frühzeitiger Diagnose konnte in mehreren Studien jedoch ein verbessertes Überleben gezeigt und erreicht werden. [17][18][19][20][21]
